Community Choice Waiver
The Community Choice Waiver provides home and community based services to eligible seniors and adults with disabilities needing assistance with personal care and daily living activities who require nursing home level of care.

Requesting Community Choice Wavier
Individuals who are interested in wavier services should contact the Request for Services Registry (RFSR) to be placed on the waiting list. The registry can be contacted by calling Louisiana Options in Long Term Care at 1-877-456-1146
What is Monitored in Home Caregiving?
Monitored in Home Caregiving (MIHC) provides 24-hour live-in care with a principle caregiver chosen by the client. The caregiver provides personal care for individuals who require assistance with at least one activity of daily living (ADL), such as bathing, dressing, eating, ambulating or toileting. Caregivers can be family members (except spouses or legal guardians) or non-family members.
Caregivers receive a monthly tax-free stipend. They also receive ongoing support and training from a registered nurse and a care manager. All caregivers must pass criminal background checks according to state regulations.
Participant Requirements
- Must be eligible for Medicaid Community Choice Waiver services.
- Must require daily supervision or assistance with one or more activity of daily living (ADL).
- Must live with principle caregiver.
Caregiver Requirements
- Must be 21 years of age or older.
- Must have high school diploma or equivalent.
- Have access to an internet capable device to complete daily electronic case notes.
- Complete annual trainings.
